Wisdom tooth pain and pericoronitis in Ireland
The most common cause of wisdom tooth pain in Ireland is pericoronitis — an infection trapped under the gum flap of a partially erupted lower wisdom tooth. Antibiotics and warm salt-water rinses calm the flare-up for a week, but if the tooth has no room to erupt cleanly, removing it (€150 to €300 simple, €350 to €500 surgical) is the permanent fix.
Why wisdom teeth flare up in your late teens, twenties and thirties, what pericoronitis feels like, and when you need urgent dental care.

What pericoronitis is and why antibiotics only buy time
When a lower wisdom tooth only breaks halfway through the gum, a pocket of gum tissue (called an operculum) sits over the back of the crown like a hood. Food particles and bacteria slide underneath that flap where a toothbrush bristle cannot reach, causing:
- A swollen, red, tender gum flap at the very back of the jaw that you accidentally bite down on with your upper tooth.
- A bad taste or smell coming from beneath the gum hood.
- Trismus (jaw stiffness) — difficulty opening your mouth wide or chewing solid food.
- Dull throbbing pain radiating into the ear, jaw angle or swollen glands under the chin.
A dentist will irrigate beneath the gum flap with antiseptic solution and prescribe antibiotics if there is spreading swelling or trismus. Because a dentist should not extract a lower wisdom tooth in the middle of an acute trismus infection (the anaesthetic works less well and the jaw cannot open wide enough), the tooth is normally removed 1 to 3 weeks later once the swelling has settled.
When wisdom tooth swelling needs same-day urgent assessment
Seek urgent same-day dental or hospital assessment if wisdom tooth pain is accompanied by:
- Swelling spreading visibly into the cheek, down the neck or around the eye.
- Difficulty swallowing saliva, speaking clearly or breathing.
- Severe trismus where you cannot fit two fingers between your front teeth, or a high fever.

Questions patients ask
Can a dentist pull an infected wisdom tooth on the same day?
For a simple upper wisdom tooth, often yes. For an impacted lower wisdom tooth causing acute pericoronitis and jaw stiffness (trismus), dentists usually flush the gum pocket and prescribe 5 days of antibiotics first so the swelling settles and the local anaesthetic can work properly.
