|---|---|---|---|

| Vertical impaction | Upright, but trapped beneath thick gum tissue or the front edge of the jawbone | Around 35% | €350 to €500 |
| Horizontal impaction | Lying completely flat (90°) sideways inside the jawbone against the second molar roots | Around 12% | €400 to €500 |

Protecting the inferior alveolar and lingual nerves
Two sensory nerves run close to lower wisdom teeth: the inferior alveolar nerve (which gives feeling to your lower lip, chin and lower teeth) and the lingual nerve (feeling in the side of your tongue). Neither nerve controls muscle movement.
Before touching a lower wisdom tooth, the dentist takes a panoramic OPG X-ray (€60 to €100). If the root tips overlap the white lines of the nerve canal on the OPG, the oral surgeon orders a 3D Cone Beam CT (CBCT) scan (€140 to €220) to see the exact millimetre clearance in three dimensions. Where the root tips are hooked directly around the nerve, the surgeon may recommend a coronectomy — removing the crown of the tooth that is causing infection while leaving the healthy root tips undisturbed in the bone away from the nerve.

Questions patients ask
Do all impacted wisdom teeth have to come out?
No. Wisdom teeth that are completely buried in bone, pain-free, disease-free and not causing decay on the neighbouring second molar are normally monitored on routine dental X-rays rather than removed preventatively. Surgery is advised when there are repeated gum infections (pericoronitis), food trapping, decay on the second molar, or cyst formation.
How long does surgical removal of an impacted wisdom tooth take?
A single impacted wisdom tooth usually takes 25 to 45 minutes in the chair from numbing to stitches. Removing all four impacted wisdom teeth under conscious IV sedation typically takes 50 to 75 minutes.
